Mod The Sims have the best tutorials for making custom content - it's not easy, but it's very rewarding once you learn how! Start here: http://modthesims.info/wiki.php?title=Si鈥?/a> and choose what you'd like to make!
It can be very confusing when you first start, so I recommend starting off with something fairly simple that can be made with free programs such as custom patterns or paintings. :)
